These FAQs explain the key parts of the Cayman Islands work permit medical process and how our service supports you. They reflect the current guidance from Workforce Opportunities & Residency Cayman (WORC), including the requirement for the Medical Declaration Cover Letter introduced in 2024. If you are unsure what applies to you, your employer or HR administrator can advise you.

Understanding Work Permit Medical Requirements

Do I need a work permit medical?

Yes. WORC requires a medical examination and a Medical Declaration Cover Letter for all work permit applications, both temporary and permanent, and for almost all renewals. WORC will not accept applications without this documentation.

If you are unsure whether you personally need a renewal medical, your employer or HR administrator can confirm this for you.

What is the Medical Declaration Cover Letter?

This is a document completed by the doctor after your examination and test results are available. WORC requires this letter for work permits and renewals instead of receiving full test results directly. This process began on 1 February 2024.

How long is a medical valid?

Under WORC’s current medical examination form, the medical and the Medical Declaration Cover Letter are valid for one year. If your last medical is older than 12 months, WORC will require an updated one for your application.

Do I need a chest X-ray?

Most first-time work permit applicants require a chest X-ray. Chest X-rays are valid for five years. If your last X-ray is older than five years, or if WORC asks for a new one, you will need to repeat it.

Your employer or HR administrator can tell you whether your application requires an X-ray.

Do renewals always need new medicals?

Renewal requirements vary, but WORC requires a Medical Declaration Cover Letter based on a medical not older than one year. Thus, the majority of people completing a renewal will therefore need an updated medical and updated blood tests. There are a few circumstances e.g. where you have had a 6-month temporary permit and are now applying for a permanent one, where it is not necessary to repeat it. Your employer or HR administrator can confirm what is required for your renewal.

Do temporary work permits require medicals?

Yes. WORC has confirmed that all Temporary Work Permit applications must include a Medical Declaration Cover Letter from 1 July 2025 onwards.

What the Medical Includes

What does the Cayman work permit medical involve?

Your medical includes:

  • Medical history review
  • Brief physical examination
  • HIV blood test
  • Syphilis blood test
  • Urine test
  • Chest X-ray if required
  • Preparation of the Medical Declaration Cover Letter

These are the tests required by WORC’s guidelines.

Do you submit results to WORC?

No. Under the current system, WORC receives only the Medical Declaration Cover Letter. All clinical results remain with you and your doctor. In our case, your work permit medical will be stored in our enterprise-level electronic patient record system to ensure its security and privacy.

About Our Service

How much does the medical cost?

We offer what we believe are the lowest-cost work permit medicals in Cayman:

  • CI$65 for the standard medical
  • CI$115 if your application requires a chest X-ray

These are total changes including all necessary blood work and the requisite letter for WORC.

How long will my appointment take?

The GP examination and testing are completed in a single visit. This usually takes a short time, typically a 30-minute appointment slot, and you can return to work immediately afterwards. If you require an x-ray, this is an extra step.

How quickly will my paperwork be ready?

The Medical Declaration Cover Letter is issued once all results are available. In all but rare cases, it is available for you to take away with you on completion of the medical process.

Can I walk in without an appointment?

We recommend booking online to guarantee your slot and avoid waits. Walk-ins are possible but may not always be available depending on existing patient load. It is definitely better to book.

Do you tell me whether I need an X-ray?

We can help you determine X-ray requirements for you, but WORC decisions may also depend on your specific application history. Your employer or HR administrator should be able to advise you. If you need an X-ray, choose the CI$115 option.

Can you send my documents to my employer or HR department?

Yes. However, this must be at your formal request due to patient privacy and confidentiality. With a formal request, we can send your completed Medical Declaration Cover Letter to your employer or HR administrator.

For Employees of Small Businesses

My company is small and does not have an HR department. Who helps me?

If your company does not have an HR department, ask your employer or supervisor to confirm whether your permit requires a chest X-ray, and whether a renewal medical is needed. If they are unsure, we can advise also, provided you accept that we will not know all of your HR circumstances or application history.

Can you remind my employer when my documents are ready?

Yes. If you request it, we can notify your employer or administrator once your paperwork is complete, if it is not already completed by the time you leave (it is in almost all cases).

For Employers & HR Professionals

Can we direct all staff to you?

Yes. Employers and HR teams can use us as their preferred clinic because the process is simple, consistent and cost-effective for their staff, locking in what we believe is the lowest rate for work permit medicals in the Cayman Islands.

Can we receive updates for our employees?

Yes. With the employee’s permission, we can confirm when their medical is complete and when documentation is ready.

Do you offer a named contact for HR teams?

Yes. HR teams receive a dedicated point of contact to streamline communication and organise medicals throughout the year.

Do you offer monthly billing?

Yes, by arrangement. For HR departments and organisations with regular recruitment, we can offer simple monthly billing by agreement. We would establish a booking process for medicals and we would invoice on a monthly basis.

Practical Information

What do I need to bring to my work permit medical?

Please bring:

  • Your passport or Cayman Islands driving licence (we cannot conduct a work permit medical without explicit proof of identity)
  • Any forms your employer or HR administrator has given you
  • A list of your regular medications

Do I need to fast?

No. Fasting is not required for the tests included in the work permit medical.

Will my employer see my test results?

No. Employers do not receive your medical results. They only receive the Medical Declaration Cover Letter if you instruct us to send it to them.

Do you keep copies of my documents?

Yes. We retain clinical records as required by law, but you are entitled to copies at any time.
